Psychosocial Hazard/incident Reporting Form

Headstart is committed to creating a mentally healthy workplace where employees have a healthy, balanced life and psychological wellbeing.

Under the Work Health and Safety Act 2011 workplaces need to prevent harm to the health and safety of workers. This includes physical and mental health where reasonable steps must be taken to eliminate these risks.

Psychosocial hazards are a serious occupational health and safety issue and can cause a person to experience a negative psychological response that creates a risk to their health. Negative psychological responses caused by exposure to psychosocial hazards can lead to physical injury, psychological injury (also known as mental injury) or both at the same time.

Headstart supports the reporting of any potential or real risks and incidents, including psychological hazards and psychological injuries.

We actively support all employees to report concerns early so that psychosocial hazards and risks are managed before serious harm occurs. Headstart will not undertake discriminatory, coercive or misleading conduct against a person who reports a psychosocial hazard or incident.

Reporting by employees of psychosocial hazards/incidents can happen in many different ways such as directly to a Co-Worker, to a manager/Leadership team member including HR, through a shift report or Quality incident report. These methods can still be used however we have provided another tool available to all employees.

Through this form you can report concerns anonymously or provide your name and choose whether or not you'd like a follow up call with the People & Culture Manager (HR) or Executive Director. Both roles have access to the information provided when reporting an incident/hazard.

This form is not to be used for reporting concerns about Consumers, in such instances please complete the incident report located here unless it has the potential/has had a negative psychosocial impact on an employee.
