Current job opening: Support Co-Ordinator/Case Worker, based in Hamilton, NSW

• 2 year contract (may become permanent dependent upon funding)
• Full time position

Headstart supports people with an ABI to pursue activities of choice in the community and re-develop independent life skills. We are seeking a motivated and resourceful person to work creatively supporting people with an ABI to become more independent in life and achieve their goals.

You will be responsible for initiating and co-ordinating new opportunities and providing a range of diverse and individually tailored supports under the NDIS for individuals living with an ABI. You will be working within a team committed to the concept of individualised supports, community inclusion and consumer driven services. This role will involve supervision and rostering of Community Support Workers and you will participate in an on-call system as required.

Your skills and experience
• Tertiary qualifications in health, welfare, or similar and completion of NDIS Worker Orientation Module ‘Quality, Safety and You’
• Minimum 2 years’ experience in case management with the ability to prioritise and co-ordinate a complex case load
• An understanding of the effects of brain injury on the psychological, social, physical and cognitive functions
• Understanding of the purpose and functions of the National Disability Insurance Scheme (NDIS) and working knowledge of the NDIS Quality and Safety Framework
• Experience in the design and implementation of supports and services around individual needs and working knowledge of the individualised planning processes
• Ability to work and communicate effectively with individuals, families and extended social supports and services
• Excellent written communication skills and the ability to prepare detailed reports and assessments with skills in MS Office
• Well developed and demonstratable interpersonal skills including leadership (support and supervision of direct staff), negotiation and conflict resolutio
• Working with Children Check
• Current National Disability Insurance Scheme Worker Check NDISWC (or be willing to obtain)
• Current Provide First Aid certificate HLTAID011 (formerly known as HLTAID003)
• Driver’s licence with access to a comprehensively insured vehicle and willingness to drive a service vehicle
• Experience in the ABI sector and community development skills are highly regarded
